Patient’s spouse attended a caregiver training session and was educated on the following: (1) functional transfers/mobility with RW and SPT to/from all household surfaces including car and tub with TTB; (2) bed mobility and pressure relief to decrease risk of skin breakdown; (3) recommendation of 24/7 assist; and (4) DME (RW, commode, TTB, and transport w/c). Patient’s spouse demonstrated ability to perform all transfers with good carryover and proper levels of assist provided, evidenced by proper use of guarding techniques and appropriate verbal cueing to maintain safety during all functional mobility within the home. Patient's spouse verbalized intent to acquire non-covered items through outside vendor and to provide 24/7 assist.
Pt’s family engaged in a verbal caregiver training session with education provided for pt’s current functional status, remaining deficits, safety awareness, and proper guarding/cueing techniques during functional mobility/transfers to maintain safety. Family confirmed comprehension of all education evidenced by return verbalization of all recommended guarding techniques and verbal cues provided to maintain pt safety. Caregiver reported providing physical assistance to pt in the past, verbalizing confidence with abilities to maintain pt safety upon discharge from HHC. In-person caregiver training is not warranted at this time, due to COVID-19 and family report of ability to maintain pt’s safety with use of proper techniques; Rehab remains available if pt’s family wishes to attend formal training in the future pending COVID-19 situation.
